I was finally able to get this to work. Had to pass --with-pam to the 
configure script and change the header include from 
<security/pam_appl.h> to <pam/pam_appl.h>

be wrong? The xinetd entry is set up as:

        wait = no
        protocol = tcp
        user = root
        server = /usr/libexec/popper
        server_args = qpopper -s -c -C -F
        port = 110
}
which I can only assume is correct.
