Local users not on the mail server are going through qpsmtpd now.
However, there are those (including me) who have accounts directly on the
server, whose email is injected directly into the queue without the
benefit of screening by qpsmtpd. If I were to implement webmail, I imagine
that would be the case, also. I suppose both local (on the server) users
and webmail users could be configured to use the server as a remote relay
or smarthost, connecting from localhost, and that would ensure qpsmtpd
checking.
