The closest thing to a documentation is the spec file in the source RPM.
It contains a list of patches and a change history. 

There is one patch which is critical for the changes in directory
layout, and there are two bug fixes (actually workarounds for a bug in
Net::DNS). The other patches aren't critical, I think. I think all of
them are in the current 0.3x branch, but I've yet to build a new RPM
using a current snapshot to check.

It runs as user qpsmtpd. If postfix is installed, that user is added to
the group postdrop. Same on debian (although I think you have to add the
user to the group manually there).
