It's not a case-sensitivity issue (C++ STL wasn't designed with Windows in mind) but I don't know the true rationale. My hypotheses include: .h is for C headers, not C++ headers; or the STL headers don't have to actually exist on disk and are free to be provided by the compiler. The truth may not be either of these.
