Looking at the error I would expect you to get an exception from the 
FormWindowW constructor complaining about e.g. failure to hook into the 
ActiveX server. The server should be registered when you set your Qt 
Jambi version in the preferences.

Could you try a couple of things for me?

1. Go into the Preferences and set your Qt Jambi version to something 
invalid (just "foobar" or something) and hit Ok. Then go back into 
Preferences again and set it to the correct location of Qt Jambi. This 
should make Qt Jambi attempt to register the ActiveX server again. Pay 
attention to whether any errors occur when you do this. Try closing 
Eclipse and reopening it and see if this has fixed the problem.

2. If this has not fixed the problem, could you try manually registering 
the ActiveX server? You can do this by first cd-ing into the 
installation directory of your Qt Jambi binary package (I take it you 
are using the Open Source package?) and then writing the following command:

    regsvr32 bin\qtdesigner.dll

This should pop up a dialog box telling you whether the registration was 
successful. If it was, could you try running Eclipse again to see if 
anything has changed?

If this issue prevents you from continuing your project until it has 
been resolved, you could also use Qt Jambi 4.3.4 together with the QJdbc 
source files that you can get from Trolltech Labs:

That might be helpful to you until your problem has been resolved.
