Windows 7 - 64, running Qt 32 bit 4.6 beta (supplied binaries). Use Visual Studio 2008 and set all debugger exceptions ON. Build the Qt sample "standarddialogs" sample. Call QFileDialog::getOpenFileName (native).
=> I get 2 exceptions in series (happening in Windows code). The program can continue safely, but I'm not sure this is normal. I don't know either if this is Qt's fault, but in another non-Qt application where the native file selector box is called, this problem does not happen. I mention above the Qt sample for reproduction. But I have the same thing in my application, which is boring when working under the debugger.
