I can reproduce this on 2 independent machines, one with OS X 10.5.8 the other with OS X 10.6.2. I have installed on these 2 machines the commercial binaries "qt-mac-commercial-4.6.0-rc1.dmg". I have no such problem with Assistant or other Qt demos using an unified tool bar. I have added the above comment to the bugreport + this screen shot:
