I have almost exhausted myself trying to figure out what is causing my smelly urine. <snip> Anyway, the antibiotic seemed to squash the smell for about a week and then it returned.

Seems like there is no one good answer (except from your anatomy that doesn't answer back in English) as to what's always going on.
I'm puzzled often too. Back when I worked full time (88-94) my urine would have a skunky smell to it. My urine made the catheter smell. I figured only I could smell it. I'd always rest my thumb on my upper thigh ON the catheter then ... and then my thumb had that smell. But NO other symptoms besides crystals/sand would clog the cath.
I wasn't on vitamins or eating or drinking any differently. The cath also turned a green or purplish color. I read in a quad magazine an article written by a urologist. He said a certain type of pseudemonas will change the cath those colors and usually a cath change will remedy it. It didn't for me. And because I had no other symptoms .. I didn't go on an antibiodic.
But! Like you said ... an antibiodic WILL get rid of it temporarily so it must be a microbial of some kind.
Then, for no particular reason, it stopped (can't recall when) -- the color change and skunky smell that is for a few yrs.
Sediment still lingered for yrs and I tried all irrigants. In around 2000 that then stopped but symtomatic UTIs started and am battling them to this day.
I'm baffled and frustrated beyond belief.
