I was surprised to discover that many knew what happened as soon as it 
happened and were awake during the trauma process.  Usually, the doctor will 
advise 
the patient of the immediate injury and then advise that the next 24 hours 
will determine what might happen.  Often, (today) trauma patients are injected 
with steroids and next drugs to prevent the spinal cord from going into dooms 
day mode.  Also, much depends on a complete vs incomplete injury.  In smaller 
hospitals, nurses are the bearer of the
bad news.  But it really varies from hospital to hospital.
