Invitation to join Study: You are invited to participate in a study
that will examine the relationship between environmental factors and
individual differences in community participation and well-being in
persons with spinal cord injury. Participation involves the completion of
four brief, user-friendly surveys that can be accessed by clicking on the
"Link to Survey" option on the side menu. www.freewebs.com/mobilitysurvey
Participant Criteria: (1) have a Spinal Cord Injury (e.g. paraplegia or quadriplegia); (2) be at least one year post-injury; and (3) be at least 18 years of age.
Surveys: Each survey question will require you to select an answer which best reflects your present experience. Once you have completed one survey, you will automatically be directed to the next survey. The surveys are as follows: (1) Demographic Information Survey; (2) the CHIEF-SF (the Craig Hospital Inventory of Environmental Factors-Short Form); the CIQ (the Community Integration Questionnaire); and the SPWB-SF (the Scales of Psychological Well-Being-Short Form).
Confidentiality: All information gathered will remain strictly confidential. No information will be shared with program providers or
product suppliers.
Completion Time: Completion time ranges from approximately twenty to forty minutes.
Assistance: If necessary, you may ask someone to assist you with survey completion. Alternatively, you may contact the researcher for a telephone survey (617-983-2021), or request that a survey package be sent to you.
Study Results: All study participants may request a summary of the survey findings by contacting the researcher as indicated below.
Stamped Envelope: Once you have completed the survey, please return it to the researcher in the enclosed pre-addressed, stamped envelope.
