I've read the other responses, yes it does very by State and area.
Funny you should bring this topic up at a time when I'm trying to start 
a modular construction co that includes leaving the ground floor open 
for 100% accessible living units. The building plan is simple, requires 
less plumbing using smart design, incorporates energy efficiency into 
the design (making it open for low cost loans or grants), and uses a 
new building material which is mostly recycled materials. All 1st floor 
units have roll in showers and sliding panel doors for second entrances 
and sliding interior doors. (Using the automatic system that works for 
sliding doors) Trying to locate them near universities with nursing 
schools to offer low rent in exchange for help. Since the manufacture 
company will be a "not for profit" the costs can be kept to the bare 
minimum. Will be looking for construction coordinators and building 
managers as soon as the first unit is being built.
I Hope to keep anyone who wants it out of nursing homes.
But part of the fight will be getting States to change the way they 
provide services.
I know those who provide the care (the aids) like my plan, as it would 
save them a lot of traveling so they could work with clients to provide 
maximum care (splitting hours etc to provide several visits over the 
day). 
They say "need is the mother of invention" and we just don't have real 
accessible housing options here.
