Pressure relief is the most important thing you can do for a problem area.  Something that I've done if I have an area of skin that has a tendency to break down is try to get the skin to keep from cracking.  I've used "Bag Balm" that the ranchers use on cows udders to keep the skin from cracking.  Sounds crazy but the lanolin in it works great!  Good luck, and stay strong! http://www.bagbalm.com/
