I use the SmartNav.  I believe it's around $300 now.  When I first bought mine it was about $130.  I have found this to be excellent to use.  If you wear glasses, you can put the reflective dot on one of the lenses and wear it all the time.  I actually have two units.  One for my desktop and one for my laptop.

I also use Dragon NaturallySpeaking 8.  I found it helpful to restart Dragon once or twice during the day.  By using the Smart Nav you can do this any time you want to.

I also use OnScreen from RJ Cooper.  This is a wonderful on-screen keyboard with excellent word prediction and complete control of the computer if you decide to use it.

The only serious problem I have with my laptop is that if it freezes I need to have someone else start it.  My desktop, however, can be restarted by using an X-10 module.
