This sounds like a laser alignment issue.  If you have a service agreement, I'd call it in.  If not, look into buying a new one.  (Fixing may be more expensive)
 
I've often had this type of issue in low-cost burners when building machines for people.
 
Go ahead and try cleaning, re-installing and also look online for any updates for the drivers and hardware.

Have you installed any new hardeware or software lately?
Have you burned a lot of CD's & DVD's? If so, when was the last good
cleaning?
If all the above don't apply, do you have the instalation software?
If not, Pioneer should have a download for it.
You can try opening "Add or Remove hardware" on your control panel,
select the drive and press remove, then reboot.
XP will recognize the drive and re-install it using their drivers.
Then you should try to find it listed under the Hardware manager where
you can find bout if it is operating properly.
If so, try a DVD.
