I have seen vendors use a wide variety of specialized foams memory, thicker sunmate foams, Roho will make an insert for our ergonomic backs and well as Stimulite.  I would highly suggest contacting your DME provider/therapists to further explore different options.  Hope this helps get things started.

In a passing dialog via email on the quad-list, at least 4 quadriplegic users of various Chairman products were found to be experiencing severe neck and back pain, possibly due to the rigidity of the seat back and its cushioning.
 
Can you offer any cushioning alternatives to possibly improve our quality of life?

I've been in a permobil for 11 years (3 chairs), and have terrible shoulder/neck/trap pain. I was attributing it to 16 years post posture problems. Perhaps we've discovered something. I'll copy you on Permobil inquiries.

THAT is what I've found to be a uncomfy feature of my, now 6-yr-old, Permobile Chairman!
 
My last chair had a VERY soft back (Fortress Scientific) and was like 3 inc pages thick of air.
 
Other chair backs don't look back-friendly either but didn't know *I* was the only one who
thought this Permobil back was hard-feeling.  I thought it was my usual severe pain.  I DID
have a vendor put more padding between the faux leather and the plastic back but made
no difference.
 
When average folks sit in my chair they find it comfy but they're not in it 10 hours either!
 
I thought it was syingomyelia pain too. 
 
Greg-how long have you had the chair?  GOD- It HASN'T been ME all these yrs of shoulder//spine/back pain? 
That's debatable.

Anyone know of a nice padded chair back that can fit on a Permobil tilt/recline Chairman?
By night time my shoulders are so sore, I can hardly think straight. I've tried egg crate cushions, pillows, etc. I was hoping to find a nice gell covered replaceable shell.
