[QUAD-L] What's in a name? Lol

I'm composing a letter to administrator but as to owner, insurance carrier, etc, I've got no idea of how to find this info out? As for her name I assume it's cause she goes around with her lip pooched out? Lol, I dunno, I've often wondered myself about that!
