Hi, I'm Paul Jacobson, a 42-year old c5/6 complete quad,
injured
November 2003. I am conducting a study with my
colleague,
Barbara Ostos (able-bodied) through the University of San
Diego's
nonprofit leadership program.
The objective of the study is to better
understand the
challenges
and issues faced by quadriplegics through
the difficult transition
from hospital to home. Please answer the 7
questions below.

1. What do you
think is the biggest challenge for those with sci when they leave the
hospital and go home for the first time?
2. What resources did you turn to upon leaving the hospital
and arriving home ?
3. How did the hospital prepare you, during
your stay and during discharge, for your transition to home.
4. What
do you wish your insurance would cover that they currently
do not.
5. When you first
left the hospital to go home, did you find any gaps
in service,
care, information, etc. If so, please share.
6. Who, in the
hospital, provided you the best information and direction regarding resources
to help you get acclimated to your new life?
7. If you were a
Board member in an organization focused on easing the transition from
hospital to home, what specifically would you like the organization to
do.
