 Request for Survey Participants
 ORAL HEALTH SURVEY
 
 The Medical University of South Carolina (MUSC) Department of
Rehabilitation Sciences is conducting a survey concerning the oral health
among people with spinal cord injury. The study is supported by the South
Carolina Spinal Cord Injury Research Fund.
 
 The survey will gather information about dental care access, oral hygiene
behavior, source of oral health information, and oral health knowledge for
people with spinal cord injury. Information from this survey will help
design an appropriate oral health program and educational materials specific
to people with spinal cord injury.
 
 The survey will be conducted by mail or completed through on-line. A
survey form or a link to the on-line survey will be sent to people who agree
to participate. All responses are completely anonymous. People who have
completed the survey will be compensated $20 (a Wal-Mart gift card) for
their time.
 
 To take part in the survey:  You must:
 1.    Be 18 years old or above
 2.    Have a spinal cord injury at least one year ago
 3.    Live in the community
 4.    Have incomplete movement in your arms, wrists, hands, and fingers
