I believe this is the the number and company I went to a fitting for a chair cushion , but when it was made I did not like, because it may pressure.  The first time I tried after bringing it home. I returned it.  They do form fitting backs also, I called them through the number listed in the new mobility Magazine article.
 
My cushion was being funded by insurance, but they were in the process of trying to get funding through Medicare.

You can look up the article in new mobility.com. They traveled throughout the country I think as they did.  They set up shop in a rehab center in Kansas City. it takes a number of long visits.

I too don't remember the name of the place.
 
I do know that Hamilton's or Hamiltons in Indianapolis does form fitting backs and cushions on the spot.  It takes a couple day to do.
