For some strange reason, that doesn't seem necessary.  Besides, you are not certified to know what you actually need.  And that might be held against you in time.  Granted, you know what you need... but you still need certification to determine that.
 
A certified Occupational Therapist is....
Possibly the Executive Director of your Center for Independent Living is....
 
Those are the first two places I would start.  After they have done an evaluation, make sure that you have a copy with your letter as proof that you really do know what you are talking about, but also have certified proof too.  I'm sure that you realize my point.
