Dear Brothers and Sisters:
You may already know national ADAPT will be in Chicago, IL from September 9
to September 13. We are writing to let you know about a forum on Sunday
September 9, where we need your help in our work to get more affordable,
accessible, integrated housing to help free our people from nursing homes
and other institutions.
Sunday September 9, from 10AM-12:30PM ADAPT will have a forum to publicize
our national housing advocacy platform to get more affordable, accessible
integrated housing for people with disabilities so we have adequate housing
in the community to go to as our work on Money Follows the Person and the
Community Choice Act moves forward.
During the afternoon from 2-4PM, officials from the US Department of Housing
and Urban Development, state housing and Medicaid officials, and other
organizations will join us for the forum. We need you to tell your story
about the problems you have had in getting out of or staying out of
institutions, with a focus on how housing barriers have forced you to go
into or stay in an institution against your will. These officials need to
understand the real, personal impact that the lack of affordable,
accessible, integrated housing has had on us, and why their support for real
housing choices makes the difference between of life and death for so many
of us.
The forum will take place at the Crowne Plaza Hotel at 733 WEST MADISON
CHICAGO, IL. If you want to testify in the afternoon between 2-4pm to help
move ADAPTs housing advocacy agenda, please fill out the attached form and

Due to the two hour time limit for the afternoon testimony session, only 30
people at most will be able to testify. We are asking that you fill out and
return the attached form so that we can find out the stories that show most
clearly how lack of housing options is forcing our people in institutions.
If you are chosen to testify we will meet with you on the evening of
Saturday, September 8 to prepare for the forum on the 9th. We must receive
the form on or before Friday, August 31 to give us time to review your
stories and get back to you in time to prepare for the forum. Even if you
are not chosen for this forum, there may be other opportunities in the
future so we urge you to fill out the form if you are interested in sharing
your story!
