Start with a medical doctor with certification in rehab medicine, to order  
an evaluation by a certified Occupational Therapist.  That completed  
evaluation goes back to your doctor for the actual order.  That order  should 
cover both specifications for the manual and electric. The rehab doctor  know 
the exact verbage and terminology to use in any written  correspondence.

My husband needs a non electric  one.  Our insurance will only pay for one 
wc every 5 years.  My  husband also has MA as a secondary.  I would like to 
know if any of you  have been able to get the insurance to buy both and how 
you did it.  Did  you have to write a letter of medical necessity?  If so, 
what was the  verbiage you used to get it approved?
