I use a Wanchik Writer http://store.ergocube.com/wanchikswriter.html
It slips over your palm and hold a pen or pencil.
I use it for my fork and spoon also, I just have to tape them on. I have 2 sets, I keep one set in my backpack incase I'm out about and want to stop somewhere to eat. They are not cheap, but work soooo much better than a velcro cuff.
