As Aaron stated, your catheters may not be the proper size.  Also, if you're 
using latex catheters you may be developing an allergy to latex.  Most of the 
major catheter manufacturers have many different models to choose from (size, 
length, amount of adhesive, non-latex, etc.)  They want your business, so 
they'll gladly send you samples if you contact them.  Also, if you get a bad 
batch of catheters, as I recently did, they'll generally replace those.

Do you use a skin prep prior to putting condom cath on?  I’ve only ever seen 
that when I have a bad UTI and my urine is breaking down the skin.
 
Another possibility is that your condom cath may be too small and when an 
occasional erection occurs the only place that can swell is the end of your 
penis.
