I've been using Roho custom cushions for years.  I have a custom Roho cushion 
for my handcycle with 2" high
cells and dual chambers (a right and left airflow valve), I use the same custom 
Roho cushion on my car seat.
For my chair I have a custom Roho Quadtro Select Hi Profile that is tapered 
from rear-to-front of my
chair (my chair is 2" wider in the front than the back).

In my opinion, unless you know exactly what you want in the custom department, 
it is important to get imput
from a seating clinic and/or seating specialist.  I have a pelvic obliquity (my 
left ischium is 2" higher than
my right ishium so all the pressure is on my left ishium)--I was thinking about 
getting a custom Quadtro with 
3" cells on the left quadrant and 4" cells everywhere else.  After speaking 
with an OT-Seating specialist I learned
that in my case it was better to go for 4" cells on the entire cushion and then 
adjust the cushion using the red/green
Isoflo valves.

I am on my second one. The Isials are suspended over the hole area of the 
cushion to reduce or help heal wounds by lessening the pressure.  Great
