I did in December. It went well and I copied the report explaining the 
procedure below. I went off warafin and did lovinox shots and I had history of 
a dvt that broke apart several times and went into my lungs. I went back on 
warafin the day after surgery. I was kept overnight and that was good because 
oral Percocet didn't cut the pain so I got slight dysreflexia and they gave me 
morphine via iv. I was released the following day and my 1st night home was 
HELL. I got sick to my stomach on my way home and dry heaved for 8 hours. my 
mom decided it was the pain medicine so we tried cutting back - BIG mistake. I 
wanted to call the ambulance but suffered through the night. I sweat my ass 
off. it was horrible. I did sit on my shower chair with the insert in while I  
showered that morning. I stayed in bed that morning after shower. Nausea went 
away and so I ate toast, took pain medicine and slept all day. I was in my 
chair the 3rd day after surgery and I sat over 12 hours......used panty liner 
to catch blood. After 4th day I was only taking pain med every 6 to 8 hours. I 
could kick myself for not doing it 2 years earlier!

SURGEON:  Glen R. Hanson, M.D.

PREOPERATIVE DIAGNOSIS:  Prolapsing grade 4 hemorrhoids with mucosal prolapse.

POSTOPERATIVE DIAGNOSIS:  Prolapsing grade 4 hemorrhoids with mucosal prolapse.

PROCEDURE PERFORMED:  Complex 3-column hemorrhoidectomy.

ANESTHESIA:  TIVO and local.

COMPLICATIONS:  None.

ESTIMATED BLOOD LOSS:  200 mL.

OPERATIVE TECHNIQUE:  The patient is a quadriplegic with some minimal sensation 
in her perineum.  I went ahead and prepped and draped the area in a sterile 
fashion with her in the lithotomy position.  Infiltrated lidocaine and then 
0.25% Marcaine.  I placed Hill-Ferguson retractor in the canal, and ellipsed 
out the left lateral, right posterior, and right anterior columns in a similar 
fashion by placing 2-0 chromic sutures at the base of each column, incising the 
overlying mucosa with a scalpel, and then resected the underlying hemorrhoidal 
tissue with a LigaSure device, resecting the hemorrhoidal tissue down to the 
internal sphincter muscles, and then closing the mucosa with interrupted 2-0 
chromics down to the dentate line and then reapproximating the anal skin with 
3-0 chromics.  The patient had all 3 hemorrhoids done in a simialr fashion, 
performing a complex hemorrhoidectomy.  She also had some small external 
hemorrhoids and skin tags that were excised and closed with 3-0 chromic, 
completing the hemorrhoidectomy.  The patient's wound was packed with 
lidocaine-soaked Gelfoam and fluffed out 4x4's.  She was transferred back to 
her room in a stable condition.

Back in my early days (about 8-years post injury--I'm a T10 complete para) I 
had several hemorrhoid "bandings"--the proctologist uses a special tool to put 
specially made, super-tight, rubber bands around the hemorrhoids, within a week 
they shrivel up and vanish.  I never had any problems.

HOWEVER, I had another friend that had a similar procedure and ended up with a 
blood clot that went to his heart--he recovered, but was fortunate he knew what 
his symptoms meant.

A couple things I've learned--make sure your doctor knows about dealing with 
SCI and how we evacuate our bowels.  I have spoken with a few proctologists 
that won't even consider dealing with SCI and hemorrhoids.

Per blood thinners--the main thing is to be monitored.  I am on blood thinners, 
and I have had to go off of them from time to time--for 3-5 days--due to 
various minor surgical procedures.  No problem, but again, it is vital to 
communicate all of this to the various medical professionals.
