Yes, a good physiatrist is like finding a gold mine. That's awesome.
Many smaller communities don't have a specialist like a physiatrist available. 
In that case, you may need to see a physiatrist remotely or travel to see one.
I'm a c5 quad, 40 years post-injury, and I've never seen a physiatrist. I have 
a general practitioner and a urologist, but no special SCI doctor. I equate 
that to two things:1. I've been very lucky and blessed with good health; no 
major issues with pain, decubitus or other injuries. I do know i could get a 
referral if needed.2. Like all of us here, I've had to educate myself about SCI 
so at least I have a general idea of what's going on with my body. Sometimes I 
have had to educate doctors.

and I have a PMNR doc that sees nothing but quads - and knows everything we 
need to know about how to navigate life to the fullest and how our bodies work! 
 She's a little further away, so I don't see her as often.  It's especially 
important to me right now, as I consider myself in an "aging quad" status.  :)
