get really thick memory foam or other material and cut out a hole. or use donut cushion with other material around it.
there is a cushion made that does have hole in middle but i think it is custom made and/or you must have company measure & build it just for you. there is no reason that so many wheelchair cushion companies cannot make a cushion that pressure maps good and prevents pressure sores. no excuses.
