Step 3 of the revised doc is probably the most effective test you can
perform... its basically starting fresh with (at that point) no scripts
added. Its just openvpn + the config. If that doesn't work then you may
be right that something in Qubes itself has become broken and maybe a
reinstall is necessary.
Before you go through the trouble of a whole reinstall, you could try
setting your VPN VM to use Fedora 28 instead to see if it works. You can
also perform a reinstall of the Debian template.
You said that Tor was running. When combining Tor with VPN, the VPN's
connection type should be TCP, not UDP. Did you check that?
When connecting to VPN through TOR, TCP is required but depending on
your provider's server config you may also have to change the port number.
