2. Tested sys-usb widget - It worked perfectly.
3. Updated Dom0 and tested widget - It worked perfectly.
4. Rebooted laptop - tested widget, but its now broken - same  diagnosis
as described above

It seems that a dom0 update has broken the widget?
Does anyone else have this problem?

Yes, I have the same problem, and find the applet useful.  Used to work
well.

Yes it was broken in -current 3-4 weeks ago. There have been many reports
and half a dozen duplicate issues on github... major snaffu.
A new version is in testing now and appears to work well - thanks Marta!
If you can't wait, I fixed my -current using:

qubes-dom0-update --action='' --enablerepo=qubes-dom0-current-testing qubes-desktop-linux-manager

I put the verbose flag on on my second try as the first was skipped due to
missing dependencies, but there appeared to be none, and it worked the second time.
