Issue 478: standardize layout header information

New issue report by vangarderen.peter:
- in addition to the 'logo' div, add a 'title' and 'description' div to the
global layout.php header
- pull in the title and description from QubitSetting values using the
'site information' scope value
- this allows them to be translated
- one issue to be solved later: syncing the app.yml setting for site title
(used in the http metas) with the Site Information settings
