New issue report by radancy:
Link institution_responsible_identifier field (in Control area) to
repository (drop-down list). Use institution_responsible's repository code
when forming full reference codes (country code + repository code +
reference code) for information objects and actor authority records. Should
be implemented before BCAUL data migrated.
Replaces issue 430.
