New issue 2158 by [email protected]: Creating accession record throws fatal
error

To reproduce this error:

1) Add new accession record
2) Add new related donor
3) Enter a new name for the donor, eg. "someone"
4) Submit donor
5) Click "create" for accession record
Resulting error:
