David - I revisited this issue to try and determine when and how it happens. And I can't make the bug happen again. At all. I tested: Create accession record and add new creator, then save. Fine. I tested: Create accession record and add two new creators, then save. Fine. I tested: Edit accession record and add a creator, then save. Fine. I tested: Edit accession record and remove a creator, then add a creator, then save. Fine.
