KOD is not the only reason why you might see .INIT. in the ntpq -p dashboard. Another potential reason is that you've never received any packets from that server (perhaps because its hostname could not be resolved at the time ntpd last restarted). So: can you narrow the problem down further? Are you sure that your internet connection is up whenever you restart ntpd? If you can't, it's perhaps a good idea to restart ntpd when your internet connection is established.
