How often do these log entries need to be made?

There are some other tools available besides ntpq.
ntptrace(8) will trace a chain of NTP servers back to the primary
source. e.g.
$ ntptrace
localhost: stratum 4, offset -0.001343, synch distance 0.011004
ntp0.kostecke.net: stratum 3, offset 0.000465, synch distance 0.072698
rover.kostecke.net: stratum 2, offset 0.000405, synch distance 0.009266
bonehed.lcs.mit.edu: stratum 1, offset 0.000007, synch distance 0.000883, \
refid 'CDMA'

You can use ntpd's logging facilities to collect various statistics. See
