This new draft fixes all but 1 design issue. The remaining issue is #96 <https://github.com/quicwg/ack-frequency/issues/96> which has an open PR #100 <https://github.com/quicwg/ack-frequency/pull/100> and was raised by Kazuho. The issue is that packet threshold loss detection may not kick in quickly or even at all if the 'Ack-Eliciting Threshold' is larger than 2. The proposed fix is a design which sends an immediate ACK upon receiving an out-of-order packet(as it does today) and then subsequently sends an immediate ACK when it can trigger packet threshold loss detection.
