Do you know if the VPS is running Ubuntu? And if it has Passenger installed as an apt package?
I believe (but I'm not 100% sure) that there is a version mismatch on the Ubuntu Passenger package where it preloads an older version of Rack. The solution, if that is the problem, is to install Passenger as a gem.
