this is actually defined by ascend as a "failure-to-start" record (it's
still sent as a Stop though). it's for when calls come in but fail to
negotiate or authenticate for some reason. most ascend stuff can be set to
avoid sending these records, it's an option like setting the "base" for
session IDs (ascends can be set to either base 10 or base 16). my TNT
radius guide talks about this on page 12-5, not sure where the
documentation would be for max's but it's probably just a matter of setting
an option in the Rad-Acct-Client part of the External-Auth profile.

I have a Radiator using running MAX4000's with what he says is the latest
software, and sometimes (not all the time) his Accounting Stops dont have a
User-Name attribute in them. This clearly plays havok with all sorts of
things.
