Hello Craig,
Looks like you are receiving your accounting requests out of order
occasionally, and that is the basic cause of this problem. So I would
concentrate on this first.
The usual cause of this is occasional lost packets between your NAS and Radius
server. I would try to investigate the reliability of packet transport between
the two.
