This is partly historical. MaxSessions was introduced first and is a hard limit
(over-riding anything else) in a Realm or Handler.
DefaultSimultaneousUse was introduced in Radiator 2.14 as a per AuthBy
parameter to work in conjunction with per-user Simultaneous-Use.
In other words, MaxSessions in a Realm or Handler will over-ride anything else,
and you must be running Radiator 2.14 or later to be able to use
DefaultSimultaneousUse in an AuthBy.
