Radiator tries to be self-healing in the face of lost Accounting Stop requests,
so every time it receives an Access-Request for a particular NAS and NAS-Port
pair it first does a delete from the Session Database for that NAS and NAS-Port
pair. This is because: first - there cannot be an existing session for that NAS
and NAS-Port pair because the call could not have come in on a busy line, and
second - the Accounting Stop for a previous session on that NAS/NAS-Port could
have been lost, so forcibly remove the bogus entry (if present) from the
Session Database.
