Use something like this:
<Realm ...>
<AuthBy FILE>
Filename %D/users
DefaultSimultaneousUse 1
</AuthBy>
</Realm>
Then in the users file for those users with more sessions allowed:
# file %D/users
someuser SimultaneousUse = 2, Password ....
....
