Here is one way to do it:

# configuration to log accounting starts and accounting stops only
# first AuthBy does accounting only (note empty AuthSelect)
# second AuthBy does authentication only (note empty AccountingTable)
# second AuthBy will also acknowledge accounting alives

<AuthBy SQL>
        Identifier SQLAccounting
        DBSource ....
        DBUsername ....
        DBAuth .....
        AuthSelect
        AccountingTable ACCOUNTING
        AcctColumnDef .....
        .......
</AuthBy>

<AuthBy SQL>
        Identifier SQLAuthentication
        DBSource ....
        DBUsername ....
        DBAuth .....
        AuthSelect select .......
        AuthColumnDef .....
        .......
        AccountingTable
</AuthBy>

# configure first Handler for accounting
# second Handler for authentication

<Handler Acct-Status-Type = /Start|Stop/>
        AuthBy SQLAccounting
</Handler>

<Handler>
        AuthBy SQLAuthentication
</AuthBy>
