It sounds like your shared secrets are not set correctly.
The radpwtst utility uses the shared secret "mysecret" by default, unless overridden by the -secret parameter. Here is the help output from radpwtst:

For your second problem, you will need to add the correct AuthColumnDef's to your configuration.
Something like this (assuming the checkattr and replyattr columns contain Attr = Value, ... lists):

If you still have problems, please send me a copy of your configuration file (no secrets), together with a trace 4 debug from Radiator showing what is happening.
