If you specify "Simultaneous-Use" as a check item that Radiator is configured to use, it will give the sort of error you see if the number of sessions is exceeded according to the Radiator session database.
You haven't included your configuration file so I can't say for sure, but if you don't want Radiator to do simultaneous use checking at all you can specify a NULL session database:
<SessionDatabase NULL>
.....
</SessionDatabase>regards

I don't have difined parameters like MaxSessions or DefaultSimultaneousUse for any realm in my config file, but some times I get the following error:
11/4/2003 7:55:25 PM Access rejected [EMAIL PROTECTED]: MaxSessions exceeded

How can I tell to Radiator that don't verify the MaxSessions for my users?
Until now, my unique solution is restart the service or use radpwtst utility.

NB: have you included a copy of your configuration file (no secrets), together with a trace 4 debug showing what is happening?
