Oh my, the grey matter is slowly slipping away. You are correct, I created the 
app about 4 years ago, most everything is inside eeps‎ and forgot the form 
style.  I probably can do what I want in a before or after start eep.

That is the representation when the form is defined as a "Variable 
Form", without
being associated with any table or view.
