Jim, is it possible that the table name got clobbered in Sys_Forms3 table?
AFTER MAKING A BACKUP, "bro all fro sys_forms3" and look for the form
name in the first column.
Check the table name in the SYS_TABLE_NAME column, correct if wrong or
missing - but note that variable forms have no table name, I suspect.
Usual disclaimer: I haven't done this, try at your own risk, your
mileage may vary, results not guaranteed.
