You've got some corruption, possibly 2 tables with the same name.

You can try any or all of these to try and resolve it:

Do a straight reload - press [Return] on any error messages. You may end up
with a good db or you may be missing a few rows from one or more tables or
even one or more tables themselves. Might be better than nothing even if you
lose a little.

Do an R:Base backup and restore. Again, try to ignore any error messages if
you get them. You might need to split your backup output file into structure
and data files rather than one big one. Try the easy way first. You'll need
to either move the original database or create a new directory for the
restored db before you run the restore(s).

If the backup and restore fails try looking through the structure part of
the output file to see if you can find any obvious errors or duplications.
If so delete the second entry. It so happens that I needed to solve the same
problem the other day when R:Base crashed on saving an amended table. (NB:
This is a  _very_  rare occurrence - just one of those things that does
sometimes happen. It was nothing to do with the saga of crashes discussed at
length elsewhere.) I ended up with 2 definitions for the same table but the
data was ok. Editing the structure saved the day. If you know what table is
the problem try searching the backup file for its name. You should easily
find the duplicate if there is one.

You might also try packing the keys only but be careful and use a copy of
the database. If R:Base can't re-create the keys you might have even more
trouble.

If none of that helps you probably need to get hold of R:Scope or a recent
backup.
