Bernie, this is not directly related to the problem you described.  However, what if someone could show you a way to store questionnaire-type responses in a table as rows with a fixed set of columns that don't vary between suvey instrument (aka a questionnaire)?  Would that be something interesting to you?  Would that be something of lasting, i.e. economic value, perhaps even pegged w/a monetary metric?
 
If any of your A's are yes, I've been developing something in fits and starts going back a bit over 3 years now to my initial concepts.  I'm w/in a few weeks of extending the development/implementation to roll it out to our (telephone) survey unit - currently, we use what's been implemented to handle data entry (transcription fm paper) and the benefits are starting to accrue.
 
Anyway, I think my design is pretty good but certainly not fully evolved.  Additionally, I'm being a bit proprietary about my data model as I think I might have something w/potential.  Nevertheless, I'd love to here your initial response and maybe we could figure out some way to work things f/our mutual benefit.
